The aim of this course is to improve patients' quality of life, support independent living skills, reduce hospital stay and readmission rates, prevent complications, and provide physical, psychological, and social support to the patient and their family.
This course covers the definition, historical development, fundamental principles and objectives, scope and organization of home patient care services; the duties and responsibilities of the home care team and its members; assessment of individuals requiring home care; care practices for the elderly, chronically ill, disabled, palliative care, and terminally ill patients; infection control, patient safety, medication management, wound care, nutritional support, and rehabilitation practices; patient and caregiver training; ethical and legal aspects; record-keeping, reporting, and documentation processes; quality management in home care services; and basic information and practices regarding the use of digital health, telehealth, and remote patient monitoring applications in home care services.
