To provide learning of knowledge, follow-up at home and providing care for chronic diseases, performing the drug applications, planning the discharge, evaluating the patient rehabilitation programs, collaboration with members of different professions and branches, education of patients and their family members to ensure effectively continuity of preventive, curative and rehabilitative care.
Content The concept of home care, Historical development of the home health care services and home health care personnel (Nurse-Health Officer) location, Home care services in our country, Institutionalization of home care services in Turkey, Home care at Health legislation, Discharge education and planning, How should be the organization and coordination of hospital services and home care services, Rehabilitation nursing.
